In the game, players take on the role of a female bull shark seeking vengeance against a fisherman who scarred her and killed her mother when she was just a pup. Maneater is a single-player open-world action RPG that puts you in control of the apex predator of the seas, where you must eat, explore, and evolve to survive.

While roaming the open world, players can uncover hidden landmarks and tackle side missions. Be wary, as various predators like muskellunge, barracuda, alligators, sperm whales, orcas, and other sharks inhabit each region. Each area also features a formidable apex predator such as a giant barracuda, shortfin mako, American alligator, great hammerhead, great white shark, orca, and albino sperm whale. Defeating these apex predators rewards players with unique skills.

The game's environment is dynamic; as the shark wreaks havoc, human bounty hunters are sent out to track it down. Successfully eliminating these hunters provides unique in-game benefits. To maintain strength and vitality, the shark must hunt and consume various aquatic creatures like fish and turtles to fulfill its nutritional needs for proteins, fats, minerals, and rare mutagenics. Players can also engage with humans by causing chaos along the coast, dismantling vessels, and disrupting water activities.
